Honor someone you love, admire and miss by participating in Good Samaritan Hospice’s Light Up A Life Celebration this season:

Please join Good Samaritan Hospice at our 18th annual Light Up A Life Memorial Service on Sunday, December 7th. This candlelight service honors our loved ones with special music and moving guest speakers; a reception will follow.
There are several ways to celebrate loved ones at the memorial service. You can submit names of those you wish to honor, to be read and shown on the large screen. There is no charge to submit a name for the service, and those remembered need not have been connected to Good Samaritan’s Hospice program.
To submit names of your loved ones, fill out and mail the linked form, or email them to mailto:evelyn.frank@goodsamhealth.org.. Please be sure to include your name, address and phone number in your email and on the form so we may contact you with questions. Names may be submitted up to December 3rd.
During this season of remembrance, Good Samaritan Hospice will also be asking for your financial support for our efforts to provide compassionate care to terminally ill patients, and for non-reimbursable services such as comfort therapies, and bereavement services to our patients and families. Please see different ways you can help in the attached form.
